Breast Augmentation (Breast Implants) in Los Angeles, CA
Breast augmentation uses breast implants to enlarge the breasts and enhance the shape and symmetry of the body. What is Breast Augmentation?
Breast augmentation refers to any cosmetic procedure, which aims to increase the size of the breasts. Breast augmentation can be used to increase the size of natural or already surgically-enhanced breasts. Breast implant procedures can change the size, shape and position of the breasts, while also allowing for symmetry, which is difficult to achieve naturally.
Types of Breast Implant (“Boob Job”) Procedures
All implants have a silicone shell that can be smooth or textured. We use smooth implant shells, as lymphoma has been associated with textured implant shells. Implants are filled with either saline (salt water) or a highly-cohesive or “gummy bear” gel. The new cohesive gel and gummy bear silicone do not migrate out of the breast capsule in the event of a rupture as did the older liquid silicone implants. Most of our patients choose gel implants as they look and feel more like natural breast tissue. Dr. Schwartz is an expert in lipo transfer or fat transfer procedures. Many women would prefer to use their own fat to enhance the breasts and Dr. Schwartz can discuss this option with you.
Choice of Incision Placement
We are able to place implants through incisions that are four cm or less.
The possible options for incision placement are:
- Periareolar (at the border of the areola)
- Inframammary (just above the inframammary fold)
- Axillary (in the armpit or axilla)
- Trans-umbilical (TUBA- through the belly button)
TUBA (trans-umbilical breast augmentation) can only be used for saline implants. We most commonly use the periareolar approach, which heals imperceptibly and does not affect nipple sensation or the ability to breast feed. A periareolar incision does not go under the nipple complex, but straight down through the breast to the lower border of the pectoralis muscle and then up under the muscle. We also frequently use the inframammary approach.
Over or Under the Muscle
The majority of implants are placed in the sub-pectoral plane beneath the muscle as it is best to have as much tissue as possible covering the implant. We recommend prepectoral or over the muscle placement in body builders, otherwise we recommend under the muscle placement.
Choice of Implant Size
Implant size is a personal preference decision. We guide each woman through this process with discussion and sizers that are shaped and can be worn under a bra to simulate the new volume of the breasts. Dr. Schwartz prefers a more natural look with breast implants, however, he is sensitive to each patient’s desires and goals and will make suggestions of implant size according to each patient’s personal preferences.
Each cup size represents approximately 150cc, so if someone is an “A” cup and wishes to be a “C” cup, generally a 300cc implant will help you achieve the size. However, it is more important to focus on your own breast and body anatomy, and determine what size implant best highlights your breast and chest dimensions. For example, if someone is a “D” cup and wants to be a “DD” we cannot use a 150cc implant as it will not fit the larger base width of that breast and the breast would be hanging off the implant. Thus, we have a detailed discussion including measurements and sizers.
Variations in Breast Anatomy
Breasts are rarely perfectly symmetrical. They are most often different in size and this can be by a minimal or a large difference. Breasts can be congenitally different in the same person, with one or both assuming a “tubular” shape, which results from a tight ring around the areola through which the breast “herniates” during development. Dr. Schwartz is experienced in recognizing and addressing these anatomic issues. We use a special technique for reconstruction of a tubular breast that includes moving breast tissue from the upper to the lower portion of the breast and altering the nipple areolar complex when necessary with a periareolar or “donut” lift. Differences in size can be addressed with different volume implants and lifts and reductions when necessary.
Recovery Following Breast Augmentation
Surgery is performed in our accredited surgery center with Dr. Schwartz, a board-certified anesthesiologist, a circulating nurse and scrub tech. On the day of surgery, you should have nothing to eat or drink after midnight the night before. Our staff will provide you with a list of everything you need to know to best prepare for the procedure.
Patients are seen the day after surgery and again at one week. Appointments are then scheduled periodically in the first few months, at one year and preferably once a year to assess the results and ensure an optimal outcome. We keep our patients’ comfort a priority before, during and after breast augmentation procedures.
Am I A Good Candidate for Breast Augmentation?
Women of all ages decide to have breast augmentation for various reasons. Whether they are in their late teens and 20s or middle age, women often desire to increase the size of their breasts. There are different reasons women choose to have breast augmentation, including if in their younger years they were naturally small or to restore fullness especially in the upper pole of the breast following pregnancy and breast feeding. There is a natural inflation and deflation of the breast associated with pregnancy and breast-feeding and although lifting improves the nipple areolar complex (NAC) position and lifts the breast, it does not restore upper pole fullness without implants.
